Ward notes, OT notes, labs, and radiology sit in different places. A doctor or medical transcriptionist has to stitch them into one NABH-aligned document before billing and TPA can move.

Cashless discharge is a file, not a paragraph. The insurer and TPA expect a consistent summary, codes that match the bill, and the attachments their desk already asked for. When ICD lines, diagnosis wording, or procedure notes disagree, the file bounces. The patient waits in a bed that is clinically free.

NABH audits punish inconsistency more than they punish a missing adjective. If cardiology, ortho, and general medicine each keep a private template, the record set looks like five hospitals. Retention rules then fail because nobody can find the signed copy two years later.
Prove it on one specialty before anyone talks annual credits.
The 7-day pilot starts with upload or scan. That is deliberate. A multi-month interface debate is how hospitals never see a signed draft. If the specialty already has a working HIS export, use it. If not, scans are enough to learn whether doctors will edit and sign.
